New Patient Forms

Tulsa Neurospecialists requires a referral from your primary care physician. Immediate scheduling is available for urgent needs.

We require certain medical information needed prior to your initial appointment:

Medical records including prior evaluations and specialty consultations focused on the neurologic concern are essential and will increase the understanding of the nature of the patient’s complaints by the physician.
Records of prior MRI, EEG, labs are important to include in prior records.
We require a patient information form that includes prior history, diagnoses, procedures and surgeries, medications, allergies, and social history of tobacco, alcohol, and illicit substance consumption.
We need your insurance information.

New patients at Tulsa Neurospecialists must complete and submit all three of these documents prior to their first visit to our practice.
